Brief proposal summary including total amount, type and length of the proposed grant.

—How does your work align with the Fund’s funding priorities?
—How would an operating grant help your organization meet its goals during the grant period?
—Does your organization have a strategic plan?
—What will be different after the grant is completed? Explain the 3-5 Measurable Results entered on your GOapply application.How does your organization approach diversity, equity, inclusion and access?
—How does your organization ensure that it does not discriminate in either policy or practice against people because of their age, race, national origin, ethnicity, creed, religion, cultural beliefs, disability or sex, including sexual orientation or gender identity and expression?
—What forms of inclusion are important to your work? How well does your board and staff leadership reflect the community that you support? Include any quantitative data that your organization has on this topic.What resources do you have and what do you need to achieve your mission?
—Human resources: briefly introduce key staff.
—Board engagement: describe the multiple ways that board members contribute to the organization (e.g., financial contributions, time, expertise, community networks, etc.). If less than 100% of board members contributed financially last year, explain why.
—Volunteers: if applicable, what is the role of volunteers in your work?
—Financial resources: what are your major categories of funding (e.g., foundation support, individual giving, public funding, earned income, etc.)? Where is your greatest need for funding?
